PATIENT INFORMATION

 

At Premier Women's Specialists, your health care is a partnership, and you play a key role in that partnership. It is important that you feel free to ask questions, and that you bring any symptoms you might have to our attention. In addition, we ask you to take an active part in leading a healthy lifestyle, including appropriate self-examinations, exercise, and proper eating habits.

 

About Your Office Visit

  • Photo ID (If address on ID does not match address in our records, other proof of identification must be shown)

  • Health Insurance Card/Information

  • A friend or family member to take notes/help with questions

  • A list of all medications you are taking

  • A list of your questions and concerns

  • Copies of records from doctors related to your visit with us

  • Completed office forms

  • A parent or legal guardian must accompany any minor patient (under age 18) for the first visit. Accompaniment for subsequent visits will be discussed during the initial visit.

How do I set up/change/cancel an appointment?

 

Call the office and we will set up an appointment. Please give 24 hour notice if you plan to cancel an appointment.

 

Do I have to pay for my visit at the time of my appointment?

 

Yes, payment is expected at time of service.

 

How much is my co-pay or co-insurance?

 

Each insurance is different. Please call your insurance if you do not know.

 

How can I get a prescription refilled?

 

Call the office during office hours so we can check your chart and please allow 48 hours to get this task accomplished. New prescriptions will require a doctor's visit.  PLEASE DON'T CALL AFTER HOURS OR ON WEEKENDS BECAUSE THE PRESCRIPTION WILL ONLY BE FILLED FOR THE AMOUNT NEEDED TO GET YOU THROUGH UNTIL THE OFFICE OPENS.

 

How can I find out the results of my tests?

 

If you do not hear from the office by phone or by mail, then please call the office and ask for the test results. NO NEWS IS NOT GOOD NEWS!

 

What do I do in an emergency?

 

If you have a life threatening emergency, call 911. If not, then call (404) 296-1424 and the service will page Dr. Daus.

 

Who should I contact if I get sick after office hours or on weekends?
